The results of county council elections in Lancashire could face a challenge in court after a blunder over postal votes, an official has told the BBC.
The numbers on some ballot papers sent out in the Wyre Borough Council area did not match the numbers on voters' declaration of identity forms.
Now, 16,000 postal ballot papers are being reprinted and sent out again.
Wyre borough council's managing director, Jim Corry, said a legal challenge was a "real prospect".
The error would mean that, when returned, the faulty ballot papers would be declared invalid and rejected.
